Your referring to my frame :)

But, I had carefully ground out the 40mm bottom bracket to 42mm --
however, it wasn't as precise as using a lathe, and yes, it causes the
bearings to prematurely fail! 

I'll post more on this later. Bottom line: Only use a KH frame, and
don't tighten the bearings too tight.
